Second time Stamp 4 renewal on basis of Irish child

Hi,

I have to renew my stamp 4 (second time) on the basis of my child being an Irish Citizen.

Are the basic documents like

GNIB, child passport, immunization card, residence proof required or anything more.

Also do we need to fill this form

NB: The Irish citizen child must attend with his/her parent(s) on this occasion.

their child's Birth Certificate
documentary evidence of the child’s address in the State (eg doctors letter or school letter)

okay, do we need any letter from the department or anything as mentioned here.
